Introduction 

This Statement of Purpose is produced in accordance with Regulation 12 of the Care Quality Commission (Registration) Regulations 2009 and the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. It outlines the aims, objectives, and details of the regulated activities provided by College Lane Surgery, ensuring transparency and compliance with CQC requirements. 

 

Our Aims

At College Lane Surgery, our aims are to: 

- Provide safe, effective, person-centred primary medical care to all registered patients. 

- Deliver services that promote health, prevent illness, and support patients in managing long-term conditions. 

- Ensure timely access to a range of primary care services delivered by qualified, compassionate healthcare professionals. 

- Uphold dignity, respect, equality, and human rights in all aspects of care. 

- Continuously improve the quality of care by monitoring performance, gathering feedback, and implementing evidence-based practice. 

 

Our Objectives

We will achieve our aims by: 

- Providing holistic clinical assessments, diagnosis, treatment, and management for acute and chronic conditions. 

- Offering preventive services including immunisations, health checks, and screening programmes. 

- Delivering accessible care through a combination of in-person, telephone, and digital consultations. 

- Ensuring all staff are suitably qualified, experienced, and engaged in continuous professional development. 

Maintaining robust systems of governance, risk management, and safeguarding. 

- Ensuring compliance with all relevant legislation, standards, and CQC regulations. 

- Working collaboratively with local health and social care partners to support integrated care. 

 

Regulated Activities Provided 

Under the Health and Social Care Act 2008, College Lane Surgery is registered with the CQC to provide the following regulated activities: 

a. Treatment of Disease, Disorder or Injury 

This includes diagnosis, treatment, management, and prevention of illness—delivered by GPs, nurses, and allied professionals. 

b. Diagnostic and Screening Procedures 

Including but not limited to physical examinations, blood tests, ECGs, cervical screening, and other approved diagnostic procedures. 

c. Maternity and Midwifery Services 

Provision of antenatal and postnatal care (Midwife). 

d. Family Planning Services 

Including contraception advice, injections, prescriptions, and related counselling.

e. Surgical Procedures

Minor Surgery and Joint Injections

 

The Services We Provide 

- GP consultations (face-to-face, telephone, online) 

- Long-term condition management (e.g., diabetes, asthma, hypertension) 

- Acute illness care 

- Minor injury management 

- Mental health support 

- Repeat prescribing 

- Vaccinations and immunisations 

- Child health surveillance 

- Chronic disease reviews 

- Sexual health advice and contraception 

- Maternity care (Midwife) 

- NHS Health Checks 

- Home visits for eligible patients 

- Health promotion and lifestyle support 

 

Patient Groups Served 

We provide services to: 

- All individuals registered with the practice 

- Temporary residents 

- Patients requiring immediate and necessary treatment 

- People of all ages, backgrounds, and care needs 

- Individuals requiring accessible, culturally sensitive, or disability-inclusive care

College Lane Surgery, Barnsley Road, Ackworth, West Yorkshire, WF7 7ZH 

Opening Hours: Monday – Friday 08:00 – 18:30 

Thorpe Lane Surgery, Thorpe AudlinPontefractWest YorkshireWF8 3HE 

Opening Hours: Monday / Wednesday / Friday 08:00 – 11:00

 

Remote consultations are provided to registered patients in accordance with safe practices and CQC guidance. 

 

Involving Patients and Feedback 

We encourage patient participation and feedback through: 

- Patient Participation Group (PPG) 

- Friends and Family Test 

- Complaints, compliments, and suggestions processes 

- Regular patient surveys 

 

Feedback is reviewed to improve care quality and patient experience.
