Reviews of Chronic Conditions

These are arranged at regular intervals with one of our Practice Nurses, at a pre-booked appointment. The interval between reviews will depend on which condition or conditions you require monitoring for, and how stable your condition is. One of these reviews will be your annual review, and may need blood to be taken to monitor, for example your cholesterol if you have heart disease. The Nurse may discuss your case with the Doctor, or arrange for the Doctor to see you, if there are any queries or problems.

Well Person Health Checks

These are offered to both male and female patients, with either a Nurse or Health Care assistant in a pre-booked appointment. They can include measurements of height, weight, BMI and blood pressure, together with advice regarding diet, exercise and weight control. 

All newly registered patients aged 5 years and over will automatically be offered a health check appointment and we would encourage them to attend this.  Please bring a fresh urine sample with you, so that it can be screened for diabetes (raised sugar levels) while you wait.

We aim to record the height, weight, smoking status and blood pressure of all our patients once a year.  If you are aged 15+ please feel free to use the automatic equipment in the waiting room at College Lane Surgery.  Please complete the form and pass this and the print out to the receptionist.

Women's Health Reviews

Female patients aged between 25 and 50 need to attend every 3 years for a cervical smear test, women aged 50 to 64 with a normal smear history need to attend every 5 years. These are done by our experienced Practice Nurses. Sometimes the time interval between smears becomes shorter than 3 years, and you will be advised of this when the result of your most recent smear is returned to you by post.

Female patients requiring contraceptive advice and prescriptions, or who use HRT treatments are also asked to attend once or twice per year a Nurse review. At all these appointments, our Nurse will also do a general health check, as detailed under “well person health checks”.

Dr Needham and Dr Moulton are pleased to be offering extended consultations to women having problems with heavy or painful periods, hot flushes or women thinking about taking hormone replacement therapy or alternative methods of managing the menopause. Please ask for women’s health appointment when contacting reception.

We have a Doctor trained to fit most sorts of contraceptive coil, and would ask for an initial assessment appointment to be arranged with a practice nurse.

Routine Vaccinations

There are groups of patients whom are eligible for several different vaccinations at different stages of their lives. The specific schedule of these changes fairly frequently, but we keep our systems up to date with who should be offered what. An example are flu jabs which are made available from September of each year to all patients aged 65 and over, and to specific patients outside this age range if they suffer from certain chronic medical conditions such as diabetes or heart disease.

Travel Advice and Vaccination

This service is provided by our Practice Nurses. We ask for as much notice before you are due to travel as is possible, ideally at least 3 months.  You will need to complete an individual information sheet detailing the itinerary of your trip before we can arrange an appointment with the Nurse.

You can collect a form from reception or complete it on line. The nursing team will advise you what vaccines are recommended for your trip.  Contact reception 5 days after you have handed in your form to arrange any necessary appointments. 

WE ARE A REGISTERED YELLOW FEVER VACCINATION CENTRE.  Some travel vaccines are not available in the NHS, you will have to pay for these.  Details of charges are available upon request.

Minor Operations and Cryosurgery

Our team of Doctors and Nurses have extensive experience at different sorts of minor skin operations, cautery, and cryosurgery (freezing with liquid nitrogen).  The technique used depends on the type of skin condition being treated.  Procedures are booked in advance during designated sessions, so an appointment may not be instantly available.

Smoking Cessation

We fully support patients wishing to give up smoking, with a local stop smoking group offered at College Lane Surgery. We ask for a simple form to be completed to register your interest, downloadable here in PDF format. Print, complete and return the form and you will be contacted with an appointment.

District Nurses

The District Nursing team offers care, support and advice for patients with acute, chronic, disabling or life threatening illness in patients own homes, in the surgery or in residential accommodation as appropriate.  We accept referrals from patients, family, cares, GP’s and other health professionals, social services and voluntary agencies for specific nursing needs such as:

The district nurses can be contacted on 01977 617400 during office hours (an answer phone will take your message) or on 01977 665886 out of hours, weekends, bank holidays or if your call is urgent).  Please note home visits are for housebound patients only and at the district nurses discretion.  If you are not house bound you will be asked to attend the surgery.

Mens Health

Many men experience erectile problems at some time in their lives and these can be both distressing and difficult to talk about, which is a shame when there are many effective treatments available. We also know that there is an increasing awareness amongst men about prostate problems but sometimes reluctance to discuss concerns.
We are pleased to offer extended appointments with Dr Hanney and Dr Eastwood to discuss these issues. This new service will start on October 1st. Please contact reception to request a male health appointment.

Weight Managment

A new practice nurse led service is available for patients trying to lose weight. This is available for those people with a BMI greater than 30, or greter than 27 with other high risk factors. BMI is a measure of body fat based on height and weight that applies to men and women. See www.nhsdirect.nhs.uk or use the scales in our waiting room to determine your body mass index. Teenage Clinic

We have a weekly parent free teenage drop in clinic for 10 to 19 year olds each Wednesday from 6.30 to 7.30, no appointment is needed. We usually have a female doctor and practice nurse available please just come along between these times if you would like to attend.
