How Do I Request a Home Visit?
Home visits are only for patients that are physically too ill to attend the surgery. Your Doctor may telephone you back to ask about further information before they visit.
Please try to phone the surgery on 01977 611023 BEFORE 10.00 am to request a home visit. If a medical emergency occurs at any other time when the surgery is open please ring us immediately.
The following information is required to request a home visit:
- Full Name
- Address
- Contact Telephone Number
- Details of Person Requesting Visit
- Exact Symptoms of the Patient
