How Do I Register?
Practice Registration Area
The practice accepts patients living in these areas:
Ackworth, High and Low
Ackworth; Badsworth; East Hardwick; Hemsworth; Kirk Smeaton; Little Smeaton; Pontefract - Larks Hill
and Brockadale estates only; Thorpe Audlin; Upton; Wentbridge and Wragby.

Please download the registration form. This is in PDF format and can be printed off and filled in. Please then present the form at the surgery. On completion of the form you will be asked to produce a form of identification which shows your address and name, e.g. Electricity bill, Gas Bill, Bank Statement.
At the Practice
To register with the practice, please collect a Patient Registration Form at reception. On completion of the form you will be asked to produce a form of identification which shows your address and name, e.g. Electricity bill, Gas Bill, Bank Statement.
